Financial Options

For your convenience we accept most major credit cards for payment. At Bloomington Oral & Maxillofacial Surgery we make every effort to provide the finest surgical care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance.

We will also work hand in hand with you to maximize your insurance reimbursement for covered procedures. As a courtesy, we will submit the initial claim to your insurance company for the services we provide you. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.


We participate with most insurance carries. To find out if we are participating provider for your insurance company, please call us at 812-323-9700 or 866-323-9701.

Sometimes it may be necessary to finance all or a portion of the fee for your services.  Financing is available through CareCredit for those who qualify.  You may apply online at CareCredit. You will be notified by CareCredit if your application is approved.
